About the Role

Responsibilities:

Ø Conduct intake risk and needs assessment with each person referred for support

Ø Contact police in emergent circumstances or potential criminal matters

Ø Support police and victim services when required

Ø Connect, liaise and collaborate with community agencies serving older adults

Ø Refer non-Elder Abuse inquiries to the appropriate community resource(s)

Ø Create and implement case plans for older adults requiring on-going support

Ø Help create safety plans for older adults at risk

Ø Advocate on behalf of the older adult accessing services and supports

Ø Create data collection and evaluation process: administer if applicable

Ø Report to stakeholders and funding bodies on a regular basis

Ø Assist in the creation of a Memoranda of Understanding and formalized partnerships

Ø Presentations and public relations regarding Elder Abuse may be required

Qualifications:

Ø Diploma or degree in Social Work or Nursing, or a relevant equivalency of education and experience

Ø Three years experience in clinical screening, assessment, case management and counselling

Ø Demonstrated ability to design and evaluate processes and protocols

Ø Demonstrated success with the development and/or implementation of community development initiatives

Ø Ability to communicate effectively to a wide variety of people, including staff members, the public and volunteers

Ø Ability to establish and maintain positive relationships with community organizations and older adults

Ø Conflict management skills

Ø Must have a working knowledge of personal computers and office productivity software

Ø Must have a Class 5 license as travel is required

Preferred Knowledge:

Ø Practical knowledge and understanding of federal, provincial, and municipal legislation and/or policies related to older adults and Elder abuse

Ø Thorough knowledge of services in the community and demonstrated skills in facilitating appropriate referrals when necessary

Hours of Work:

Ø This position may require work outside regular business hours, based on client need.

Depending on reporting, this job is up to 25 hours per month
