Senior Supports and Systems Navigator

Full time Term Position (minimum 32hours) April 1, 2025 – March 31, 2026

Starting at 24.95 or 26.02 with credentials and experience

As a Senior Supports and Systems Navigator, your focus is enabling seniors to remain in their home as long as possible by supporting and inspiring doing what they love to do. In collaboration with Healthy Aging Alberta and Accredited Supports to the Community (ASC) our goals are to professionally collaborate and access community services to engage in organized senior centered service planning.

The successful candidate will participate in the Healthy Aging Provincial Community of Practice and oversee the promoting of ASC’s Connect Seniors Supports Program. This role also includes receiving referrals and going to in home visits to ensure individualizing the correct supports for accessing community health and social resources to meet the needs of each senior.

With the direct support of ASC Senior Supports and Systems Program Coordinator the ideal candidate will have the skills to:

· Facilitate access and application for financial benefits and assistance

· Complete records and documentation accurately and thoroughly

· Evaluate and report on the programs project activities and outcomes

· Assist seniors with accessing mobility devices

· Liaison emotional support to ensure healthy habits and social engagement

· Ability to ensure safety and comfort, which includes promoting the independence and mental well-being of the person in a respectful and compassionate manner

· Provide services to seniors in their homes which can include driving and accompanying to appointments, errands, shopping and outings, meal preparation, light housekeeping, and/or provide companionship

· Outreach support as needed by assisting with the organization of household moves and the downsizing process

The successful candidate will provide

· Achieved a post-secondary credential in the Social Services sector

· Applicants should have at least 2 years experience in Human Services and management experience would be an asset

· Two managerial/supervisor references from your most current employment

· Skilled in Microsoft Office Applications

· Strong verbal and written interpersonal communications skills

· Ability to maintain confidentiality and professional boundaries

Requirements:

· Standard First Aid/CPR Certification

· A reliable vehicle, class 5 driver’s license, 2 million liability insurance for work purposes

· A Vulnerable Sector Criminal Record Check issued by the RCMP or City Police
